America Health Care Capital provides assistance to Medicaid-eligible individuals who are in nursing facilities to successfully relocate to their community of choice. We have a team of relocation services who can help our clients return safely to their homes through the provision of appropriate community-based and in-home services.

To be eligible for Transition Assistance Services, an individual must be currently living in a nursing home but is able to return to their residence with proper services and must be Medicaid-eligible. They must also be enrolling in one of the following Medicaid waiver programs upon being discharged from the nursing facility: Medically Dependent Children Program, Community Living Assistance and Support Services, and Deaf-Blind with Multiple Disabilities.
