The Primary Home Care (PHC) program is intended to provide home care services to Texas residents who need assistance with activities of daily living such as bathing, grooming, dressing, and toileting. It is available to eligible Medicaid clients whose health conditions limit their ability to perform daily tasks.
To be eligible for the PHC program, you must be 21 years old and above who requires assistance with at least one of the activities of daily living (bathing, grooming, eating, dressing, or toileting). Your physical limitation must be due to a medical condition and is documented by a qualified medical professional.
PHC services include:
- Assistance with Bathing
- Assistance with Grooming and Personal Hygiene
- Assistance with Dressing
- Assistance with Eating
- Assistance with Toileting
- Light Housekeeping
- Meal Preparation
- Medication Reminders
- Transportation Assistance
