Through our Consumer Managed Personal Attendant Service (CMPAS) Program at America Health Care Capital, we contract with licensed home health agencies that allow us to provide services to adults aged 18 years old and above with physical disabilities. To qualify, you must be emotionally and mentally competent. You must also be willing to oversee the personal attendant or have someone who can oversee the attendant for you.

Through this program, we provide our clients with the opportunity to interview, train, supervise, and discharge their selected personal attendant (unless you appoint someone else to do it for you). Note: Any and all health-related tasks will be prescribed by your personal physician.

Eligibility requirements:

• Must be 18 years old and above
• Must need help with at least one personal care task
• Must be capable (emotionally and mentally) of directing their own care
• Must have a physical disability that is expected to last at least six months from when the eligibility date is determined
